You need to be informed of your rights as a patient under the Health Insurance Portability & Accountability Act of 1996 (HIPAA).

Under HIPAA,  understand that your personal information may be used to:

• Provide and coordinate your treatment among a number of healthcare providers who may be involved in your treatment directly or indirectly

• Obtain payment from third-party payers for your healthcare services

• Conduct normal healthcare operations such as quality assessment and improvement activities

 

Understand that you may request in writing how your private information is used or disclosed to carry out treatment, payment or health care operations. Also understand that Red Rocks Psychiatry LLC is not required to accept your requested restrictions, but if they are accepted then understand that Red Rocks Psychiatry LLC will honor your request unless it is an emergency.
